Facility Reservation Agreement for Fitness Studios

Complete this form if you would like to reserve the Fitness Studios for a private practice or a private group fitness class lead by a W&M Fitness Instructor.

THIS FORM MUST BE SUBMITTED AT LEAST 5 WORKING DAYS PRIOR TO YOUR EVENT. LATE REQUESTS MAY NOT BE HONORED.

COMPLETING THIS FORM DOES NOT GUARANTEE AVAILABILITY OF THE FITNESS STUDIOS.

RENTAL FEES
There is no fee to reserve a fitness studio. However there is a fee to have a Fitness Instructor present if you wish to organize a special private group fitness class for a group or organization.
We charge $30/hour per Instructor. You may pay in cash or check. Checks must be made out to William and Mary Rec Sports. You must make your payment before the group class. Please take payments to Jenny Ruehrmund's office in the Student Recreation Center.

CONDITIONS OF AGREEMENT
Place your initials next to each item to confirm you have read and agree to the rental terms.
   
1. All participants entering the Rec Center must show a valid William and Mary ID or pay the $5.00 guest fee.
   
2. Food and beverages are not allowed in the Fitness Studios (except for water).
   
3. Alcoholic beverages are NOT allowed. Any individual who shows signs of alcohol use will not be allowed to enter the Rec Center.
   
4. The Recreational Sports Facility Supervisor reserves the right to deny admittance should he/she feel that it could jeopardize the safety of the participants.
   
5. No black-soled shoes can be worn in the fitness studios.
   
6. No fitness equipment (including the stereo and mic) can be used unless a Fitness Instructor is present. Your group is permitted to bring a portable stereo or CD player if you need music.
   
7. The fitness studios must be left just as you found them. Please clean up after yourself and your group. If anything is damaged in the fitness studio while your group is in there, please let the Rec Center front desk supervisor know immediately.
   
8. Once your reservation has been confirmed, you will be able to go to the Rec Center front desk to request access. A facility supervisor will let you into the studio. When you leave, make sure that the door is locked and lights are turned off.
 
9. For private group fitness classes, each individual in your group must sign an informed consent form. This form will be provided by the Instructor at the time of the class.
 
LIABILITY
The College of William and Mary is not responsible for injuries/accidents through the voluntary participation in its Recreational Sports program or the use of its facilities. It is strongly recommended that all participants have a satisfactory health status (through an annual physical examination) and maintain personal accident/health insurance available through the college.
   
FACILITY USE

Use of any specific facility does not imply universal facility use/availability to your group. In cases of inclement weather, groups scheduled to use the facility may still use the space as long as the Rec Center is still open.

I, as the person named responsible for this event, have read, understand and agree to abide by and enforce the conditions stated in this agreement. I also agree to be present for the duration of the event. For violations of the Recreation Center policies/procedures, individuals are subject to penalties ranging from warning to dismissal. Organizations that fail to follow the policies or procedures may be denied the privilege of scheduling future events in addition to possible sanctions against the organization.
