weeks, so please make this appointment as soon as possible. You
must
download and print the following forms for you and your physician to

at your appointment.

Step 2: Health Insurance Information

Step 3: Student Health Center
Insurance Information

IF YOU HAVE EXISTING
HEALTH CARE COVERAGE for your child and DO NOT wish to purchase
the
student health insurance coverage offered through the Student Health
Center at The College of William and Mary, you need to visit
http://www.wm.edu/health/insurance to submit a waiver request.
The
link for the waiver request form is under waiver information on the
left side of the screen. YOU MUST
SUBMIT THIS ONLINE! If you do not submit this request by July
1st,
you will be automatically enrolled in and billed for coverage
under the student insurance administered by United Health Care. Please
note that this basic plan does NOT cover costs associated with athletic
injuries.

IF YOU DO NOT HAVE INSURANCE
COVERAGE, you may obtain coverage at www.esurance.com/home/health.asp
If you are uninsured when you enroll in the College of William and
Mary, you will be automatically enrolled in and billed for coverage
under the student insurance administered by United Health Care. Please
note that this basic plan does NOT cover costs associated with athletic
injuries.

Step 4: IF APPLICABLE New prescription requirements
for the NCAA
The National Collegiate Athletic
Association (NCAA) bans certain classes of drugs because they can harm
student-athletes and could create an unfair advantage in competition.
The NCAA will grant medical exceptions if adequate documentation
showing the student-athlete has undergone a diagnostic evaluation for a
type of drug. Exceptions may be granted for substances included in the
following classes of banned drugs: anabolic agents, stimulants, beta
blockers, diuretics, anti-estrogens, and peptide hormone. If you are on
these medications, CLICK HERE for
a letter to give to your doctor to obtain the proper documentation for
your medication so that we may have it on file in the Athletic Training
Room. The NCAA now requires us to have this information on file.
